The three kinds of “wrinkle”, and why it matters

Lumping every line together is the most common reason treatment disappoints. There are really three different things going on, and each has a different answer.

Line types and what actually helps
TypeWhat it looks likeMain causeWhat helps most
Dynamic linesAppear when you frown, smile or raise your brows; smooth out at restRepeated muscle movementAnti-wrinkle injections (botulinum toxin)
Static linesVisible even when your face is stillCollagen and elastin loss, sun damage, years of dynamic creasingSkin boosters, polynucleotides, microneedling, resurfacing; sometimes a trace of filler
Folds & creasesDeeper grooves (e.g. nose-to-mouth), worse when volume is lostLoss of deep fat and bone support, skin laxityDermal filler placed for support, HIFU/RF for laxity — not “filling the line”

Ask this at your consultation

“Is this line dynamic, static or volume-related?” A good practitioner will show you on a mirror. That single answer tells you which treatment is worth your money.

What causes fine lines and wrinkles

Ageing skin is normal and not a problem to be fixed. But several factors accelerate visible lines, and most are modifiable.

  • Sun exposure (photoageing). UVA breaks down collagen and elastin. This is the largest single controllable factor.
  • Repeated expression. Decades of frowning or squinting etch dynamic lines into static ones.
  • Collagen decline. We lose roughly 1% of dermal collagen per year from our mid-twenties, faster around menopause.
  • Volume and bone changes. The facial skeleton and deep fat pads change shape with age, so skin has less to sit on.
  • Smoking, poor sleep, dehydration and rapid weight change all show in the skin.
~80% of visible facial ageing is attributed to UV exposure rather than the passage of time itself. Widely cited photoageing research; see Sources

Anti-wrinkle injections: how they work and what to expect

Botulinum toxin temporarily reduces the signal from nerve to muscle in the treated area. The muscle relaxes, the overlying skin creases less, and dynamic lines soften over one to two weeks. It does nothing for sun damage or lost volume.

Commonly treated areas

  • Frown lines (glabella) — the vertical “11s” between the brows
  • Forehead lines — horizontal lines from raising the brows
  • Crow’s feet — fanning lines at the outer eye
  • Bunny lines, a gummy smile, downturned mouth corners, and neck bands, in experienced hands

The appointment, step by step

  1. Assessment and prescription

A prescriber reviews your medical history and examines your movement patterns. Anti-wrinkle injections are prescription-only in the UK.

  2. Marking and treatment

Tiny amounts are injected into specific muscles with a fine needle. It takes about ten minutes and feels like quick pinpricks.

  3. Aftercare

Stay upright for four hours, avoid heavy exercise, facials and alcohol for 24 hours, and don’t rub the area. Normal activities otherwise.

  4. Two-week review

Results settle by day 14. A good clinic includes a review to add a small top-up if a muscle needs a little more. This should not cost extra.

What “frozen” looks like — and how to avoid it

An over-treated forehead loses all movement and can drop the brows. It comes from too high a dose or poor placement. Conservative dosing keeps natural expression; you should still be able to show emotion.

Treating static lines: skin-quality treatments

Lines etched at rest need the skin itself to be rebuilt — more collagen, better hydration, improved elasticity. These treatments work gradually, over a course, and are then maintained.

Skin-quality options for fine, static lines
TreatmentWhat it doesCourseDowntime
PolynucleotidesStimulates fibroblasts and repair; improves fine crepiness and quality3 sessions, 2–3 weeks apartMinimal — small bumps for hours
Skin boostersMicro-deposits of hyaluronic acid; deep hydration and glow2–3 sessions, then 4–6 monthlyMinimal — tiny bumps, mild bruising possible
Medical microneedlingControlled micro-injury triggers collagen3–6 sessions1–2 days redness
Radiofrequency microneedlingAdds heat for stronger tightening and remodelling3 sessions2–3 days redness/swelling
Medical peelsResurfaces the outer layers; brightens and smooths3–6 sessions, seasonalLight flaking for a few days

For the periorbital area (under-eye fine lines), gentler options are usually chosen — polynucleotides or a carefully selected booster — because the skin there is thin and reactive.

Why combination plans work best

The face doesn’t age in one dimension, so treating one dimension rarely satisfies. A typical, effective plan for someone in their forties bothered by forehead and eye lines might look like this.

Example first-year plan
MonthTreatmentPurpose
0Consultation + anti-wrinkle injections (frown, forehead)Soften dynamic creasing
1Skin booster — session 1Hydration, fine-line quality
2Skin booster — session 2Build result
3Skin booster — session 3Complete initial course
4Anti-wrinkle injections (repeat)Maintain
8Anti-wrinkle injections (repeat)Maintain
OngoingDaily SPF + retinoidProtect and prevent

Indicative first-year total ≈ £900–£1,300. Year two, with the booster course behind you, is usually £500–£800.

How long results last

Typical duration of effect
Anti-wrinkle injections 3–4 months Skin boosters 4–6 months Polynucleotide course 6–12 months Dermal filler (support) 9–18 months RF microneedling course ~12 months

Durations vary with the individual, the product and the area. Maintenance keeps results steady rather than letting them fully fade and rebuilding.

Prevention: the cheapest anti-wrinkle plan there is

  1. Daily broad-spectrum SPF 30–50, every day, all year. Non-negotiable.
  2. A retinoid at night (start low, build tolerance). The best-evidenced topical for fine lines.
  3. Vitamin C in the morning for antioxidant support.
  4. Don’t smoke, and moderate alcohol.
  5. Sleep and hydration. Unglamorous, but visible.
  6. Consider a small amount of preventative anti-wrinkle treatment in your thirties if dynamic lines are starting to etch in — “prejuvenation”.

Safety, regulation and choosing a practitioner

  • Anti-wrinkle injections are prescription-only medicines — a qualified prescriber must assess you in person before prescribing.
  • From 2025 the UK is tightening the rules on who can perform non-surgical cosmetic procedures; check your practitioner’s registration (GMC, NMC, GPhC or GDC) and their aesthetic training.
  • The clinic should be a clean, appropriate medical environment with proper consent, record-keeping and a plan for managing complications.
  • Be wary of heavily discounted “parties”, mobile-only injectors with no clinic base, and prices that seem too good to be true.

Frequently asked questions

What is the best treatment for fine lines and wrinkles?

It depends on the type of line. Dynamic lines from movement respond best to anti-wrinkle injections; static, etched lines need skin-quality treatments such as polynucleotides, skin boosters, microneedling or resurfacing; deeper folds from lost volume need support restored with filler or laxity treated with HIFU or radiofrequency. Most people do best with a combination.

How much does treating fine lines and wrinkles cost in London?

A single area of anti-wrinkle injections is around £180–£280. A first-year combination plan (a couple of areas plus a skin-booster course and skincare) is typically £450–£1,300. Maintenance years are lower because the initial courses are behind you.

How long do anti-wrinkle injections last?

Usually 3–4 months. The effect builds over one to two weeks and then wears off gradually. Regular treatment keeps the result steady; some people find the interval lengthens over time.

Will I look frozen or expressionless?

Not with conservative dosing and precise placement. The aim is softer lines with natural movement. A frozen look comes from too high a dose or poor technique. Starting low and reviewing at two weeks avoids it.

Can fine lines be treated without injections?

Yes. Medical skincare (SPF and a retinoid), microneedling, polynucleotides, skin boosters and peels all improve fine, static lines without any injectable muscle relaxant. They work gradually over a course.

What can I do at home to prevent wrinkles?

Daily broad-spectrum SPF is the single most effective step. Add a night-time retinoid and a morning antioxidant, don’t smoke, and look after sleep and hydration. These do more over a decade than any single treatment.

Is there any downtime?

Anti-wrinkle injections have essentially none — occasional tiny bumps or a small bruise. Skin boosters and polynucleotides may leave tiny bumps for a few hours. Microneedling means one to two days of redness; radiofrequency microneedling two to three days.

At what age should I start?

There is no “right” age. Some people start small preventative treatment in their thirties as dynamic lines begin to etch in; others start later when static lines appear. A consultation will tell you whether treatment is worthwhile for your face now.

Are anti-wrinkle injections safe?

When prescribed and administered by a qualified, trained practitioner after a proper assessment, botulinum toxin has a long safety record. Side effects are usually mild and temporary. Risks rise sharply with untrained injectors and unregulated settings.

How do I choose a clinic?

Look for a registered healthcare professional (GMC, NMC, GPhC or GDC) with specific aesthetic training, a proper clinic base, clear consent and records, honest pricing, and a plan for managing complications. Avoid discounted group events and prices that seem too low.
